Job Description:
 

This position is located at an off site location located in Mineloa, Long Island. The position coordinates the provision of hospital services for patients of affiliated practices, and serves as a liaison between the referring physician, patient and Mount Sinai Hospital departments in addition to the Long Island Heart and Good Samaritan Hospital.

Serves as the point of contact for physician referrals and obtains necessary information to initiate the patient's referral to Mount Sinai Medical Center.

Coordinates the referral to the appropriate Mount Sinai provider(s), sets up necessary appointments and communicates with patient and authorized family members.

Assists patients and families in all aspects from the point of referral to follow up after discharge in navigating Mount Sinai Medical Center facilities.

Advocates for patients, families and referring physicians to promote overall satisfaction with Mount Sinai Medical Center experiences.

Promotes communication and collaboration between Mount Sinai Medical Center and the affiliated practice to improve patient outcomes and promote overall satisfaction with Mount Sinai Medical Center experiences.

Performs other duties as assigned.

Job Qualifications:
 

High School Diploma/ GED
Associates or Bachelors Degree preferred.
Minimum of 2 years o experience in a Hospital or Clininal Setting.
Knowledge of Medical Terminology
Ability to work in a fast paced and high volume environment.
Excellent customer service and communcation skills.
Microsoft office applications.
